Aboriginal and Torres Strait Islander Health Assessment
The practice is situated in the suburb of Kareela, within the Sutherland Shire. This region is part of the Dharawal Nation, which traditionally extends from the south side of Botany Bay and the Georges River down to the Shoalhaven River and inland towards the Campbelltown and Camden areas. Kareela Family Practice values and celebrates the Dharawal culture and language, and acknowledge the Dharawal people's continuing connection to the land, the sea, and the community.
What is the Aboriginal & Torres Strait Islander Health Assessment ?
A free, comprehensive health check designed to support your long-term wellbeing. The 715 Health Assessment is a Medicare-funded, holistic health check for Aboriginal and Torres Strait Islander people of all ages. It is designed to support early detection, prevention, and ongoing care tailored to your individual needs. At our practice, this is not a rushed check-up — it is a dedicated appointment focused on your whole health, including physical, emotional, and social wellbeing.
What the 715 Health Check includes
Your doctor will take time to understand your health through a full and respectful assessment, which may include:
- A review of your current health and medical history
- General physical checks
- Discussion of lifestyle factors such as nutrition, smoking, alcohol, and activity
- Mental health and emotional wellbeing review
- Medication review and safety checks
- Immunisation status review
- Screening for chronic conditions such as diabetes, heart disease, and kidney health
- Assessment of hearing, vision, and general function (where appropriate)
- Discussion of family, social supports, and daily wellbeing needs
Where appropriate, we may also organise follow-up tests, referrals, or care planning to support your ongoing health.
Why this health check matters
Many health conditions develop quietly over time. The 715 Health Check helps identify risks early — when they are easier to manage or prevent. This includes conditions such as:
- Diabetes
- Heart disease
- Kidney disease
- High blood pressure
- Respiratory conditions
- Mental health concerns
Early detection allows for early support — helping you stay well, active, and connected to care.
What to expect at your appointment
We aim to make your visit comfortable, respectful, and unhurried. Your appointment will usually include:
- Time with your GP (and sometimes a nurse or health worker)
- A private and confidential discussion about your health
- Physical checks and simple tests
- A clear explanation of any results or follow-up needed
- A plan for ongoing care if required
You are welcome to bring a support person.
What type of appointment should I book?
A 715 Health Assessment requires a long appointment ( 30 minutes) with your GP. This appointment length allows enough time to complete all Medicare requirements for a comprehensive health assessment, including a full review of your physical health, wellbeing, and relevant medical history. It also ensures there is time to discuss your health concerns without feeling rushed, ask any questions you may have, and talk through your overall wellbeing in a relaxed and supportive setting. Where appropriate, this time also allows us to develop a clear plan for follow-up care, further investigations, or ongoing support if needed. Our aim is to ensure you feel heard, informed, and supported throughout your visit.
