A 75+ year old health assessment is a Medicare funded appointment designed to support older adults in maintaining good health, independence, and quality of lift. This is a long, dedicated appointment with your GP and nurse, focused on prevention, early detection, and planning care that supports you as you age.
What the 75+ Health Assessment includes
Our GP and Nurse will take a holistic approach to your health, which may include:
- A full review of your current physical health
- Assessment of blood pressure, heart health, and general wellbeing
- Revew of medications and possible side effects or interactions

- Falls risk assesment and mobility review
- Hearing and visions concerns

What type of appointment should I book ?

Dedicated time with each of the Nurse and GP ensures there is enough time to discuss your health in detail without feeling rushed, answer any questions you may have, and talk through your needs in a calm and supportive setting. Our aim is to ensure you feel heard, informed, and supported throughout your visit.
Who can have a 75+ Health Assessment ?
This is a Medicare bulkb billed health assessment is available for patients aged 75 years and over. It can be updated once every 12 months.
What to expect at your appointment ?
With our practice nurse
Our nurse will take time to:
- Review your general health and medical history
- Check you blood pressure and basic health measurements
- Review your medications
- As about your day-to-day wellbeing, mobility, and supports
- Identify any concenrs for further review
Your GP will then:
- Review your overall health and assessment findings
- Discuss any medical concenrs in more detail
- Check for early signs of illness or age-related changes
- Review chronic conditions and current treatments
- Talk through next steps, if needed.

How long does the appoint take ?
This is an extended time in the practice to ensure your health is properly reviewed. It usually takes 45 minutes with the nurse and another 30 minutes with your GP.
Do I need to prepare anything before my appointment ?
You don't need to do anything special. It can be helpful to write down any questions or concerns you have may, and bring a list of your current medications.
Can I bring someone with me ?
Yes. You can bring a family member or support person with you to your appointment.
