A 45–49 Health Check is a Medicare-supported preventative health assessment designed to identify early risk factors for chronic disease such as heart disease and diabetes. This is a structured, team-based assessment, involving both our nursing team and your GP.
How your health assessment works
To ensure a thorough and accurate assessment, your health check is completed in two stages:
Nurse-led assessment
- A review of your medical history and lifestyle factors
- Blood pressure, height, weight, and general health measurements
- Blood tests and pathology requests arranged by your GP
- ECG (heart tracing), if clinically appropriate
- Initial risk screening for cardiovascular and metabolic conditions
- Identification of any health concerns for GP review
GP review and health planning
- Review your test results and clinical findings
- Assess your overall health risk profile
- Discuss any early signs of chronic disease risk
- Talk through your lifestyle, goals, and priorities
- Develop a preventative health plan tailored to you
- Document your health assessment outcomes as required for Medicare
- Plan any further investigations or ongoing care if needed

Do I need to prepare anything for my appointment ?
No special preparation is needed. It can be helpful to bring a list of medications, any recent test results, and any questions you may have about your health or lifestyle.
What appointment should I book ?
This health assessment involves both our Nurse and your GP. The simpliest way to book appropriately is to telephone the practice and let reception know you want the 45-49yr old health check. Reception will ensure the right appointment time with the nurse and GP are booked.
How much is the 45-49yr old health check ?
This health check is funded by Medicare. The assessment will be bulk billed for patients with a valid Medicare card and have registered KFP as your preferred health provider. If other clinical issues arise during the consultation, or additional tests are needed, these may be privately billed.
